Family Services Care Coordinator is responsible for assessing the family’s needs and strengths, identifying appropriate services, enhancing supports and ensuring that referrals are made to appropriately matched services and supports. Work with families with children and youth who are at risk for abuse and neglect, who have serious emotional disturbance (SED) or a developmental disability (DD) and/or who have juvenile corrections involvement in the home and/or school setting. Responsible for the functions of Wraparound Facilitator, including family engagement, facilitating the wrap process, writing and revising the family care plan and scheduling the facilitating care plan team meetings. May also directly provide services to families they are not assigned as the Wraparound Facilitator to avoid dual relationships with an appropriately reduces caseload, when credentialed, trained, or authorized through their agency to do so and with supervision.

Must be willing and able to work a flexible schedule including evenings and weekends as needed.

Requirements: Wrap Facilitator Functions, including:

Family Engagement:  build trust and orient family members to the Wraparound  process; Facilitate Wraparound process; Support self-efficacy o families and Enhance  natural supports; Support and facilitate with the family the Wrap Planning Team membership, integration and success; Connect families to culturally relevant community –based resources and supports; Schedule and facilitate Family Care Plan and Wrap Planning Team meetings. Transport clients and family as needed.

Family Assessments: With supervision, conduct bio-psycho-social and strength-based, culturally competent family needs assessments.  Arrange for or administer other assessment tools as needed, such as what is required for the RI Positive Educational Partnership initiative for the duration of that project identify emergency or urgent needs according to protocols including safety, risk and caregiver protective capacity factors. Review legal and ethical issues (mandated reporting laws and client’s rights).

Documentation Management: Obtain or ensure collection of required records (e.g. evaluations, assessments); Maintain required program documentation and individual child/family records;
Obtain HIPAA consent, including privacy notice. Document revise and develop with family the family care plan and provide copies of plans to families and other parties as required, with family authorization; Obtain documentation of required releases and ensure client confidentiality; Complete data collection as required for system evaluation Administer the functional assessments for program evaluation such as the North Carolina Family Assessment, Ohio Scale, and Modified Children’s Global Assessment Scale; Managed documents required for the RI Positive Educational Partnership initiatives for RI PEP-involved families for the duration of the project.

Service Provision, Coordination, Referral: Link and refer families to appropriate services and informal supports and secure and initiate services identified in the Family Care Plan; Coordinate team members to ensure that all are invited, included and engaged in the Wraparound Planning Team; Ensures the Family Care Plans include transition plans that support family members and maintain positive outcomes; Carry  emergency beeper in rotation; Establish emergency protocols with families Educate and/or role-model developmentally appropriate child rearing practices; Make referrals for meeting unmet basic needs such as for housing, food, clothing, medical and transportation Make referrals for job training programs or child care; Provide in-home or agency-based direct/clinical services and interventions for families for which they are not providing wraparound facilitation.
Advocacy: Advocate for families during the family care planning process and coordinate wraparound process from a strength-based perspective; Ensure that families and others (e.g. agency representatives) understand the Wraparound process; Follow-up with Wraparound Planning Team members on assigned tasks resulting from Family Care Plans; Track progress to ensure that the Wraparound principles are upheld in all aspects of family care planning; Assist families in accessing parent advocates or peer mentors.  Any FSCC with experience raising a child with SED or DD and knowledge/competencies needed to effectively support another parent or caregiver may also perform the role of Family Support Partner to assigned families in order to support their effective participation in the high fidelity wrap around process. No FSCC will function as both the Wraparound Team Facilitator and the Family Support Partner for the same family.

Education: Bachelor degree
